You are not required to consent to receive SMS messages as a condition of receiving care at our practice.

Message frequency will vary based on your appointment schedule and administrative needs. Standard message and data rates may apply depending on your mobile carrier and plan. Cognitive Works, PLLC is not responsible for any charges incurred from your mobile carrier.

5. How to Opt Out

You may opt out of our SMS program at any time by replying STOP to any message you receive from us. After opting out, you will receive a single confirmation message and no further messages will be sent. You may re-enroll at any time by replying START or by contacting our office.
